Here are the two most important forms you will need as part of the marriage process - the Notice of Intended Marriage (Form 13) which kickstarts the process of getting married, and the application form for a Registered Marriage Certificate (QLD weddings only) needed for proof of identity and marriage in the future.

Notice of Intended Marriage (Form 13)

The Notice Of Intended Marriage is the document that kickstarts the process of getting married. It must be lodged with a registered celebrant no later than one month and one day ahead of the proposed wedding date, or no earlier than 18 months ahead of the proposed date. (If you need to marry in a hurry, there are special lodgment conditions that may apply to you - talk to me about this). Couples must complete this document in their own handwriting, and initial any corrections made by themselves or the celebrant.

Application - Registered Copy of Marriage Certificate(QLD)

If you or your partner intend to change names after marriage, you need to complete one of these forms and send it with the prescribed payment (currently $34) to the Registry of Births, Deaths and Marriages QLD after the wedding to obtain the registered certificate you need for name-change purposes.
